About Forge & Form Therapy
Forge & Form Therapy was built for people who are done with surface-level work and ready to go deeper.
Founded by Sean Maloney in the heart of Rosslyn, the practice brings together a team of clinicians with distinct backgrounds and a shared conviction: that real change requires precision, honesty, and a relationship you can trust.
We work with teens (12+) and adult individuals, couples, and all relationship structures across the full range of what brings someone to therapy. Whether you are navigating a major transition, sitting with something that has never quite resolved, or simply aware that the way you have been moving through your life is no longer working, we are here for the serious, substantive work of change.
Our approach is integrative and intentional. We do not apply the same framework to every person, and we do not mistake activity for progress. We take the time to understand who you are, what has shaped you, and what you are actually trying to build, and then we work toward that with you in a way that is grounded, direct, and specific to you.
Forge & Form is private pay, and we work with clients in person in Arlington and via telehealth across Virginia and Washington, DC. We offer a free 15-minute introductory call for anyone considering whether this is the right fit.
Frequently Asked Questions
-
We work with individuals, relationships, and professionals who are navigating transition, relational strain, or a deeper sense that something in their life no longer fits. Many of our clients are high-functioning and insightful, but feel stuck, disconnected, or at odds with the lives they've built. If you're ready to engage with depth and intention, you're in the right place.
-
We specialize in identity development, relational conflict, burnout, perfectionism, high-performance stress, and major life transitions. Our work often focuses on clients who are doing “well enough” externally but feel misaligned internally—and want to do something about it.

Forge & Form Therapy is a private-pay practice and does not participate in insurance networks. However, we are happy to provide a superbill that you can submit to your insurance provider for potential out-of-network reimbursement.
If you would like to contact your insurance provider to determine your benefits for out-of-network therapy, here are some questions for you to ask them:
Do I have mental health benefits?
What are my benefits for out-of-network therapy services?
What is my deductible for out-of-network services and has it been met?
How much will insurance reimburse me for out-of-network therapy services (for CPT codes 90834, 90837, and 90791)?
Do I need pre-authorization or a referral from my primary care doctor in order to see a therapist?

Individual and couples sessions are typically 45 minutes. Most clients begin with weekly sessions, but frequency may shift depending on your goals and progress. Intensives and group formats, including nontraditional relationships, vary in length and structure.
-
The best way to find out is to schedule a brief free consultation call. There’s no pressure. The goal is to explore whether working together feels aligned with what you’re carrying—and what you want to move toward.
-
Therapy tends to focus on emotional patterns, relational dynamics, and personal history. Coaching is more future-focused and action-oriented, often centered around identity, leadership, and purpose in the context of work and creativity. We’ll determine together which approach fits best—or whether a hybrid model serves you.
-
Yes. We offer secure, HIPAA-compliant virtual sessions for clients located in Virginia and other states where we are authorized to practice. Coaching services may be available across state lines.
-
Relationship intensives are immersive, high-impact sessions designed to help you move through stuck dynamics quickly. We slow the pace, go deeper, and structure the time to focus on what really needs attention. Each intensive is customized and begins with a short assessment to shape the agenda.
-
No—we do not prescribe medication. Our work focuses on talk therapy and relational process, not pharmacological treatment.
If we decide that medication might be a helpful part of your care, we can refer you to a psychiatrist, psychiatric nurse practitioner, or primary care provider who specializes in medication management. We are also happy to coordinate with your prescriber to support an integrated approach to your care.
-
In the course of our work together, we form a diagnostic impression — a clinical assessment based on what emerges in session — and the associated diagnosis code is what's used when we provide a superbill for insurance reimbursement. This is standard practice in psychotherapy.
What we don't provide is a formal, comprehensive diagnostic evaluation: the kind of in-depth assessment (often involving structured testing) used for purposes like legal proceedings, school accommodations, or disability determinations. If that level of formal evaluation is what you need, we're happy to refer you to a trusted psychologist or psychiatrist who specializes in diagnostic assessment.

Your privacy is a top priority at Forge & Form Therapy. All sessions and records are kept strictly confidential in accordance with federal and state laws, as well as professional ethics. We do not sell any personal information under any circumstances. Communications—including phone calls, emails, and SMS messages—are handled through secure, HIPAA-compliant systems and cannot be sold or used for marketing purposes. We collect only the minimum information necessary for licensing compliance and to build an effective therapeutic relationship: your name, contact information, and date of birth. Information you share will not be disclosed without your written permission, except in cases where I am legally or ethically required to do so—such as if you are at risk of harming yourself or others, or if there is suspected abuse or neglect of a child, elder, or dependent adult.
Proven Expertise
Forge & Form brings together four clinicians with distinct backgrounds and a shared commitment to doing real work with the people who seek them out.
Sean Maloney, LCSW, founded the practice after fifteen years that took him from international NGO work in post-conflict regions to direct clinical practice in the DC area. That path gives him a depth of perspective on trauma, meaning, and human resilience that is difficult to teach in a classroom. Lexie Beckstrand, LPC, works with adults navigating transitions of identity, relationship, and belonging, particularly those who appear to have it together on the outside while sitting privately with harder questions. She brings psychodynamic grounding, genuine warmth, and a belief that good therapy can also be human and even funny. Taylor Blackburn, LPC-R, specializes in high-achieving adolescents and young adults who are running hard and starting to feel the cost of it. She helps clients slow down long enough to understand what is actually driving them, and to define success on their own terms. Dr. Claudia M. Carrera, PsyD, is a licensed clinical psychologist and Army veteran who works with anxiety, PTSD, insomnia, and complex mental health concerns, and conducts psychological assessments. She is fluent in English and Spanish and brings particular experience with military, federal, and Intelligence Community populations.
Together, the team offers individual therapy, relational work, psychological assessment, and coaching across a range of modalities, tailored to each client. We see people in person in Arlington and via telehealth across Virginia and DC.
